Hi Hunt Club, my name is Nurse Randi and I'm your school nurse! I joined the Hunt Club staff during the 2013-2014 school year and haven't looked back since. Mrs. Bonnie Spevere will also be back in the nurse's office with me in the Health Office Secretary position. We look forward to another great school year! KINDERGARTEN AND NEW/INCOMING STUDENT HEALTH REQUIREMENTS
--State of IL Physical Exam Form + Up to Date Immunizations due by September 15, 2024
*Your student(s) may face exclusion from school if the correct physical/immunization forms are not turned in by this date.
*Physical forms must be dated 8/15/2023 or after
--Eye Exam Form due by October 15, 2024
--Dental Exam Form due by May 15, 2025
Forms can be brought to school, sent in with your student, emailed, or faxed to the contact info below! Please do NOT upload forms to PowerSchool as we may be unable to access them on our end.
SD308 Health Forms & Documents
MEDICATION AT SCHOOL
The SD308 Medication Authorization form must be completed and signed by a doctor and parent before a medication can be given at school. This applies to ALL medication, prescription and over the counter including: Acetaminophen, Motrin, eye drops, allergy medications, antibiotics, inhalers, cough drops, etc. Medications MUST be transported to/from school by an ADULT, not your student.
WHEN TO KEEP YOUR CHILD HOME FROM SCHOOL
-Vomiting and/or diarrhea within last 24 hours
-Fever (greater than 100.0)
-Shortness of breath or wheezing, not controlled with an inhaler (if applicable)
-Constant or uncontrolled cough/coughing that wakes child in the night
-Rash that has not been evaluated/cleared by physician
-Drainage + redness from one or both eyes
-Severe sore throat
